I currently collect and process the following information:

How I get the personal information and why I have it

Most of the personal information I process is provided to us directly by you for one of the following reasons:

To enable Fine Fettle Foot Care to contact you.

I may share information with your GP or Podiatrist, with your permission, such as for referrals for example if ulcers are found, requiring more advanced reviews.

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases I rely on for processing this information are:

1) Your consent. You are able to remove your consent at any time. You can do this by contacting [email protected]

2) I have a legal obligation.

How I store your personal information

Your information is securely stored.

I keep information such as;- Full name and address, date of birth, telephone numbers/email address, name address and telephone number of your GP, date of consultation, any relevant medical conditions such as diabetes, arthritis, renal problems, heart or circulatory conditions, dermatological disorders, epilepsy, etc. and any medication you are currently taking, for the duration of your treatment and for twelve months after your treatment has ended. I will then dispose your information by deleting from the electronic data base.

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.

Your right to object to processing - You have the right to object to the processing of your personal information in certain circumstances.

Your right to data portability - You have the right to ask that I transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond to you.
